2023 Pride Consent Form

Hi Friends!

Thank you so much for being a part of our 14 year milestone, celebrating the work of Rivets and Roses and the love of each of you! If you are here, it means you have been a big part of our life and business and we are so grateful for your participation in this project!

Below is a general model release for you to sign - thank you so much!

Rivets and Roses | Consent + Model Release 

For good and valuable consideration, receipt and sufficiency of which are hereby acknowledged, I hereby:

1. Consent to have my name, portrait and likeness recorded (via photography, collectively the “Photographs”) by, on behalf of, or at the direction of Photogen Inc., d/b/a Rivets and Roses (“Publisher”);

2. Irrevocably grant Publisher, its licensees and assigns, the non-exclusive, transferable, royalty-free, perpetual right and license, throughout the universe, to use, reproduce, distribute and display my name, voice, portrait and likeness, in and in connection with the purpose of publishing the 2023 Pride Press Release ("Release"), created on behalf of Rivets and Roses. The Photographs will be used in the PR media efforts for Rivets and Roses and used for any and all purposes (whether in a commercial or editorial context) in any and all media, formats and methods of transmission now known or hereinafter developed, without any restrictions as to changes or alterations (including but not limited to composite, blurred, distorted or computer altered representation) to promote the Release;

3. Agree that Publisher owns the copyright of the Photographs and I hereby waive any claims I may have based on any (a) right to inspect or approve the finished version(s) of the Photographs, including any written copy or other materials that may be created and/or appear in connection therewith; and (b) usage of the Photographs or works derived therefrom, including but not limited to claims for invasion of privacy, libel, trademark infringement, copyright infringement, commercial appropriation or violation of the right of publicity; and

4. I have read this Release and I understand its contents and that it shall be governed by and construed in accordance with the applicable laws of the State of Minnesota without giving effect to the principles thereof relating to conflicts of laws. I am 18 years of age or older and competent to sign this release.
